For this month, we have chosen\u00a0the municipal homecare as our focus.\u00a0The municipal home care is experiencing a number of errors, which are presented in a previous report from the\u00a0Patient Safety Board.<\/p>\n

The home care\u00a0use\u00a0Kibodan\u2019s\u00a0Medimax, which can store\u00a0a large number\u00a0of pills.<\/p>\n

We had a talk\u00a0with Jannik, who work in the municipal home care. We get into\u00a0subjects like the daily care, the use of\u00a0Kibodan\u00a0products, and how mistakes may occur.<\/p>\n

Do you briefly <\/b>want to\u00a0<\/b>tell about yourself\u00a0<\/b>and your duties and what you do?<\/b>\u00a0<\/b><\/h2>\n

My name is Jannik and I’m 24 years old.\u00a0I am what they call a\u00a0social and health assistant and\u00a0finished\u00a0my education 2 years ago,\u00a0and\u00a0then\u00a0I\u00a0started in\u00a0the\u00a0Home Care.<\/p>\n

What does\u00a0<\/b>your day look like when you clock in<\/b>\u00a0Monday morning?<\/b><\/h2>\n

I clock in\u00a0early\u00a0Monday morning, then\u00a0there is a morning meeting where we go through the plans\u00a0for the week. We review what happened\u00a0the\u00a0last 24 hours, or maybe longer if you\u00a0have had a vacation or\u00a0a\u00a0long weekend. Then we go through what to do the rest of the day, and\u00a0then we\u00a0prepare\u00a0the medicine schedules,\u00a0for example,\u00a0update them in case\u00a0we have to dose\u00a0later in the day, so that\u00a0you are updated even before\u00a0you\u00a0go and visit the\u00a0citizens.<\/p>\n

How many citizens do you typically have in a day?<\/b><\/h2>\n

It depends a lot on the visits, some visits take 10-15 minutes, while other visits take an hour or two. It varies a lot. We also have\u00a0the entire task\u00a0with citizens as an assistant, so\u00a0we should also\u00a0be able to\u00a0help them with a bath\u00a0when the need arises. So it depends\u00a0what the patient or citizen needs, but usually, six in the morning and three to four in an afternoon, but that varies\u00a0a lot from day to day.<\/p>\n

Do you know them all personally, do you have any regular citizens or how does it work?<\/b><\/h2>\n

Yes, we have regular citizens. There has been a lot of talk about what is the best, but we have assessed that permanent citizens\u00a0are\u00a0the best solution,\u00a0so that the citizens get to know us, we get to know the citizens, and we get a much better approach and can be allowed much more.<\/p>\n

They feel safe, and we also feel that some personnel\u00a0are not allowed to do so much unless you are the regular person, and you are allowed to get a little more\u00a0done because they more feel safe with one regular person.<\/p>\n

What do you think i<\/b>s your biggest challenge in one\u00a0<\/b>day?<\/b><\/h2>\n

Sometimes, we are time-constrained\u00a0because in\u00a0the homecare, the plan is planned\u00a0before each visit, and it may take a little bit more time, but for the most part it fits\u00a0and we have time to do our tasks, but people are good at helping each other if you were to run out of time.<\/p>\n

What do you enjoy most about your day?<\/b><\/h2>\n

All the citizens who are happy to see me and the help I\u00a0give them.<\/p>\n

What do you think are the most important values \u200b\u200bwhen you work in\u00a0<\/b>the\u00a0<\/b>home care?<\/b><\/h2>\n

Being able to see\u00a0things\u00a0from the citizen\u2019s\u00a0point of view.\u00a0When you visit the\u00a0patients,\u00a0you see yourself in\u00a0very strange situations and some very pressured situations, because you do not know what to do right away, but you just\u00a0have to remain calm and keep your head clear.<\/p>\n

It’s easy enough, when\u00a0you do the same tasks\u00a0everyday, but most often you\u00a0get involved in situations you\u00a0aren\u2019t\u00a0prepared for.<\/p>\n

Have you\u00a0<\/b>ever\u00a0<\/b>experienced that\u00a0<\/b>patients have been given wrong medication<\/b>?<\/b><\/h2>\n

Yes, I feel that there is a challenge with\u00a0that, not exactly wrongful\u00a0medication, but challenges if they are getting their medicine from the doctor\u00a0at all,\u00a0ie.\u00a0a mixture between us (home care professional), doctor and pharmacy. Then the medicine\u00a0is not delivered on\u00a0time, or\u00a0it\u2019s\u00a0not delivered for a few days. It\u00a0has\u00a0happened once or twice in my two years.<\/p>\n

Is it due to the time pressure that lies there?<\/b><\/h2>\n

I think\u00a0it’s a mixture\u00a0of communication errors and the system. It is typically\u00a0the IT system and\u00a0that\u00a0there is an error that you\u00a0are not\u00a0aware of, and you cannot track it.<\/p>\n

We have to call and\u00a0ask\u00a0the pharmacies and hear why the citizens\u00a0cannot get their medicine.<\/p>\n

There is another thing,\u00a0when people come to the pharmacies and they pick up their medicine. They\u00a0don\u2019t always\u00a0get all their medication because there is some medicine on some old prescriptions, and pharmacies only provide what has just been ordered, and not the\u00a0ones from\u00a0the old prescriptions.<\/p>\n

So it’s not the responsibility of the patients to get their medicine?<\/b><\/h2>\n

Yes, the citizens must get the medicine\u00a0themselves, but it is us who order it for those who cannot. We would prefer the citizens to pick up the medicine themselves, or order it and collect. If they\u00a0do not, we will do that for them. We do it so they can keep the shape, their way of life and their activity.<\/p>\n

It\u00a0<\/b>must<\/b>\u00a0also\u00a0<\/b>be\u00a0<\/b>a big challenge, keeping track of the right medicine<\/b>?<\/b><\/h2>\n

Yes, and then you come to dose at the\u00a0citizens home, and then they have not picked up their medicine, and then they have to pick it up\u00a0the following day, but maybe\u00a0that is on the weekend, so there are a lot of little things\u00a0that\u00a0can go wrong.\u00a0If we miss one day,\u00a0things might go\u00a0very\u00a0wrong.<\/p>\n

It is also nece<\/b>ssary to have a good overview<\/b>?<\/b><\/h2>\n

Yes, definitely. An overview of the medicine, it is very important as it may change from time to time.<\/p>\n

How does the dosage work?<\/b><\/h2>\n

It works so that\u00a0in the morning we print the schedule.\u00a0We have been told that now is the day to\u00a0dose. Then we\u00a0visit\u00a0to the citizen who has the medicine in their home, the old medical schedule\u00a0in the home, and then we have\u00a0the new medical schedule\u00a0with\u00a0us, and of course we will dose\u00a0after the new medical schedule,\u00a0check if there have been any changes.\u00a0If there have been no changes, then we just dose as the schedule says. If there have been any changes, we will go in and look at\u00a0it\u00a0,if there has been a simple box from the end to be extra dosed or removed.<\/p>\n

How often does it appear that you have to postpone if there has been a mistake?<\/b><\/h2>\n

In making mistakes, it is very rare. I do not even\u00a0know if I can remember that\u00a0has happened. But because we need to postpone\u00a0the dosage,\u00a0because some new medicine has come from a doctor. We are dosing for 14 days at a time, and if we have just been there and dosed and then the doctor doses\u00a0some new medicine, we will\u00a0have to\u00a0visit\u00a0the citizens\u00a0again\u00a0and put their new medicine in their boxes. It also happens that the citizen has not obtained any medicine, and you only get\u00a0dosed for three days, with some of the medicine, you should also\u00a0visit them again.\u00a0But we would rather avoid post-dosing.<\/p>\n

Do all your citizens use\u00a0Kibodan\u00a0products?<\/b><\/h2>\n

Yes, it is also a requirement we have. At least if we are going to dosage for them, then it should be them.<\/p>\n

Why is\u00a0Kibodan’s\u00a0dosing boxes so necessary? Could it be done differently?<\/b><\/h2>\n

It can easily be confusing. They are\u00a0very clear, your boxes. Morning, noon, evening, bedtime.\u00a0It is quite clear\u00a0and when you open them you cannot risk getting the medicine\u00a0mixed up. They are transparent, and it is also an advantage. For us who dose the medicine, because we can count and fit, but also for the citizens to see whether the pills are taken or not. The\u00a0days are displayed on top, which they obviously should be.<\/p>\n

How often do you use the ex<\/b>tra box? In the\u00a0Medimax<\/b>, we have 8\u00a0<\/b>boxes. How often do you use the extra, for the<\/b>\u00a0PN medicine that it is used per. Need.<\/b><\/h2>\n

Yes, we also use that with some citizens.\u00a0Because\u00a0the citizens themselves have to take them\u00a0so\u00a0we write\u00a0PN\u00a0Panodil\u00a0or Tramadol for these occasions, and then\u00a0the home help can take from there.<\/p>\n

What is important when sitting and dosing medicine? What are the most important values \u200b\u200bhere?<\/b><\/h2>\n

It is important with some quiet, good light, good working condit<\/code>ions. The medicine must be bought and ready and\u00a0that it is the correct medicine, of\u00a0course. That the medical\u00a0schedule\u00a0has been updated\u00a0is important as well.<\/p>\n

Do you have a regular routine that you go through when you come and visit the citizens?<\/b><\/h2>\n

Not such a regular routine, I have normal courtesy, say hello, explain what\u00a0I\u00a0do, reviewing what I do, so that the citizens are also aware of what\u2019s\u00a0happening.\u00a0It\u2019s\u00a0important they do not\u00a0feel you take responsibility\u00a0for everything. I would like to have them inward even though\u00a0it’s actually me who does all of the work.<\/p>\n

Are you satisfied with our products and what do your colleagues think?<\/b><\/h2>\n
